Details of the Response

1. What approximate percentage of your patient records – i.e. scans, letters, notes and results – are currently digitised?

The Trust does not hold this information.

We have an electronic document management solution (Kainos Evolve), which we introduced in 2016. The legacy paper medical records of all current patients have been scanned (so-called On Demand Scanning) and all paper forms generated during a patient attendance or episode are scanned into the Evolve solution (Day Forward scanning).

2. Is your NHS Trust planning to digitise all patient records?

None of the above.

Legacy paper medical records for patients who have not been since pre-2016 are stored in accordance with retention schedules – we have no plans to digitise these.

3. How many paper-based patient records did your NHS Trust collect in Q4 2018 (October, November and December)?

The Trust does not hold this information.

4. Are members of staff receiving training on how to manage digitised patient records?

Yes.
